In this guide
Colorado child support modification forms, and the other family court forms used to change an order, are published by the Colorado Judicial Branch. Each one carries a JDF number and a revision date. The table below describes what each form is and links to GoFile's guide where one exists. It does not tell you which forms to file; the official instructions and the process guides describe each path.

Where official forms live
The official source for Colorado family court forms is the Colorado Judicial Branch at coloradojudicial.gov. Its self-help forms library lists each form by number. Its change child support page groups the forms used for a child support modification. Many forms also have an instruction sheet, marked with an "i" after the number, such as JDF 1403 i.
How to tell a form is current
Every Judicial Branch form shows a revision date in its header or footer, marked "R:". JDF 1821, for example, carries a March 2026 revision date. To check a copy, compare its revision date with the copy on coloradojudicial.gov. Third-party form sites sometimes serve older versions.
The 2026 child support law is a clear example. Before March 1, 2026, support was calculated on Worksheet A or Worksheet B. Those worksheets were replaced by the single Support Worksheet, JDF 1821. A worksheet on an old form reflects the old calculation. GoFile tracks revisions in its Colorado forms updates page and in monthly posts such as the May 2026 forms update. The worksheet change itself is covered in JDF 1820 discontinued and JDF 1821 renamed.
E-filing or paper
The Judicial Branch lists two ways to file a modification. You can file through Colorado Courts E-Filing, or in person with the court that entered the original order. The fee list shows a $105 filing fee for a motion to modify filed more than 60 days after the order. E-filing adds a separate per-filing fee, posted on coloradojudicial.gov.
Fee waiver forms
JDF 205, the Motion to Waive Fees, asks the court to waive court fees. The form lists filing fees and e-filing and e-service fees among the fees a court may waive. The Judicial Branch's fee waiver page pairs it with JDF 206, the court's order on fees. The court decides whether a waiver is granted.
